RESUMO

The aim of this study was to evaluate the effect of two finishing techniques, glazing or polishing, in comparison with the as-cut condition, on the biaxial-flexural-strength (BFS) of a zirconia-reinforced lithium silicate ceramic (ZLS). Cylinders were milled from CAD/CAM blocks and sliced to obtain disc-shaped specimens (ISO6872:2015). Polished and glazed specimens were processed following the manufacturer's instructions. Thirty-three specimens were obtained for each condition and microstructural and BFS/fractographic characterizations were performed. BFS and roughness data were analyzed using Weibull statistics and ANOVA one-way with Tukey post-hoc test, respectively. While a rougher surface was observed for as-cut specimens, smoother surfaces were observed for polished and glazed ZLS at microscopical evaluation and confirmed through surface-roughness evaluation. X-ray spectra depicted a glass phase for all groups and characteristic metasilicate, lithium disilicate, and lithium phosphate peaks for the as-cut and polished specimens. Glazed specimens showed higher characteristic strength than polished and as-cut specimens, which did not differ significantly. While higher Weibull-modulus was observed for the polished than for the as-cut specimens, no statistically significant differences were noted between glazed and polished, and between glazed and as-cut specimens. ZLS presents higher strength when glazed, and polishing increases the structural reliability of the material relative to the as-cut condition. Both finishing techniques reduced surface roughness similarly.

RESUMO

The use of identical prosthetic components for all implant diameters could reduce the production costs by companies and the complexity of component selection for the clinician and his team. However, it would imply in reduction of thickness of the cervical walls of tapered internal connection implants, which could compromise the reliability of narrow and extra-narrow implants. Therefore, this study aims to evaluate the probability of survival and failure modes of extra-narrow implant systems with the same internal diameter as standard-diameter implants using the same prosthetic components. It was used eigth different implant system configurations, including narrow (Ø 3.3 mm) (N) extra-narrow (Ø 2.9 mm) (EN) and extra-narrow-scalloped (Ø 2.9 mm) (ENS) implants, both with cementable abutments (Ce) or titanium bases (Tib) and one-piece implants (Ø 2.5 mm and Ø 3.0 mm) (OP) (Medens, Itu, SP, Brazil), comprising the following groups: OP 3.0; OP 2.5; N Ce; N Tib; EN Ce, EN Tib, ENS-Ce and ENS-Tib. The implants were embedded using polymethylmethacrylate acrylic resin in a 15 mm matrix. Standardized maxillary central incisor crowns were virtually designed and milled to fit on the different studied abutments and cemented using a dual self-adhesive resin cement. The specimens were submitted to SSALT (Step Stress Accelerated Life Testing) at 15 Hz in water until failure or suspension of the test, until a maximum load of 500 N. Fractographic analysis of the failed specimens were realized in scanning electron microscopy. All implant systems demonstrated high probability of survival (90-100%) for missions at 50 and 100 N and values of characteristic strength superior to 139 N. Failure modes were restricted to the abutment in all the implant configurations tested. Therefore, the use extra-narrow implants with standardized prosthetic components for different implant diameters is a viable option for the replacement of anterior teeth.

OBJECTIVES: To assess the probability of survival of crowns made with a 3Y-TZP, a 5Y-TZP, and a lithium disilicate. MATERIALS AND METHODS: CAD-CAM premolar crowns with occlusal thicknesses of 1.0 mm or 1.5 mm and cemented on a metal dye. Step-stress accelerated life testing (SSALT) was performed to access the use level probability Weibull curves and reliability were calculated for the completion of a mission of 100,000 cycles at 300 to 1200 N. RESULTS: All ceramics showed a high probability of survival (87-99%) at 300 N, irrespective of thickness. 3Y-TZP shows no significant decrease in the probability of survival up to 1200 N (83-96%). Lithium disilicate presented lower reliability than zirconia under the 600 N mission. 5Y-TZP showed lower reliability than 3Y-TZP at 1200 N. There was no significant difference in the Weibull modulus (3.23-7.12). 3Y-TZP had the highest characteristic strength (2483-2972 N), followed by 5Y-TZP (1512-1547 N) and lithium disilicate (971-1154 N). CONCLUSION: Zirconia ceramics have a high probability of survival (up to 900 N-load), while lithium disilicate survives up to 300 N load only, irrespective of the thickness (1.0 mm or 1.5 mm). CLINICAL RELEVANCE: The probability of survival of posterior crowns made with zirconia ceramics resists extreme loads while glass ceramics resist normal chewing loads. In addition, crowns with thinner occlusal face showed sufficient mechanical behavior.

OBJECTIVE: An important factor affecting the biomechanical behavior of implant-supported reconstructions is the implant-abutment misfit. This study evaluated the misfit between Ti-Base abutments and implants by means of polyvinyl siloxane replica technique using microcomputed tomography (µCT). METHODOLOGY: Volumetric and linear (central and marginal) gaps of four Ti-base abutments (n=10/group): (i) Odontofix LTDA (OD), (ii) Singular Implants (SING), (iii) EFF Dental Components (EFF), and (iv) Control Group (S.I.N implants) compatible with an implant system (Strong SW, S.I.N Implants) were measured using µCT reconstructed polyvinyl siloxane replicas. RESULTS: The results showed significantly lower volume gap for Control S.I.N (0.67±0.29 mm3) and SING (0.69±0.28 mm3) Ti-base abutments relative to OD (1.42±0.28 mm3) and EFF groups (1.04±0.28 mm3) (p<0.033), without significant difference between them (p=0.936). While gap values were homogenous in the central region, EFF presented a significantly higher marginal gap. Accordingly, the Control S.I.N and Singular Ti-base abutments showed improved volumetric and marginal fit relative to Odontofix and EFF. CONCLUSION: The method of manufacturing abutments influenced the misfit at the implant-abutment interface.

PURPOSE: To evaluate the pullout resistance of CAD/CAM implant-supported crowns cemented with provisional and definitive cements on Ti-base implant abutments. MATERIALS AND METHODS: Sixty crowns were milled for use in Ti-base implant abutments and divided (n = 15/group) according to material, as follows: (a) [Pr] Temporary acrylic resin; (b) [Co-Cr] Cobalt-Chromium alloy; (c) [Zr] polycrystalline zirconia; and (d) [Ti] titanium. The cementation was performed with RelyX Temp NE (RxT) cement or RelyX U200 self-etching resin cement, under a 50 N (5 kg) load for 10 minutes. Twenty-four hours after cementation, the crowns were subjected to the pullout test in a universal test machine, at a 1.0 mm/min crosshead speed. The tests were performed first without cement to evaluate frictional resistance (Baseline), then with provisional cement (RelyX Temp NE without cement again (Baseline After RxT), and finally with resin cement (U200). The results were analyzed by ANOVA and Tukey test (p < 0.05). RESULTS: Data evaluation as a function of cement type demonstrated the superiority of resin-based cements relative to provisional and baseline groups (p < 0.01). While Co-Cr crowns presented the highest pullout strength values, Pr showed the lowest values (data collapsed over cement) (p < 0.001). Retentiveness data as a function of both factors demonstrated similar pullout resistance between groups without cement (p < 0.001), except Zr baseline. Also, Co-Cr presented higher pullout strength compared to other materials. CONCLUSIONS: Self-adhesive resin cement exhibited superior retention compared to temporary cement, regardless of crown material. Co-Cr and titanium presented higher levels of retention to Ti-base abutment after being cemented.

OBJECTIVE: In this study, marginal and internal misfit and fracture load with and without thermal-mechanical aging (TMA) of monolithic ZLS and lithium disilicate (LDS) crowns were evaluated. MATERIAL AND METHODS: Crowns were milled using a computer-aided design/computer-aided manufacturing system. Marginal gaps (MGs), absolute marginal discrepancy (AMD), axial gaps, and occlusal gaps were measured by X-ray microtomography (n=8). For fracture load testing, crowns were cemented in a universal abutment, and divided into four groups: ZLS without TMA, ZLS with TMA, LDS without TMA, and LDS with TMA (n=10). TMA groups were subjected to 10,000 thermal cycles (5-55°C) and 1,000,000 mechanical cycles (200 N, 3.8 Hz). All groups were subjected to compressive strength testing in a universal testing machine at a crosshead speed of 1 mm/min until failure. Student's t-test was used to examine misfit, two-way analysis of variance was used to analyze fracture load, and Pearson's correlation coefficients for misfit and fracture load were calculated (α=0.05). The materials were analyzed according to Weibull distribution, with 95% confidence intervals. RESULTS: Average MG (p<0.001) and AMD (p=0.003) values were greater in ZLS than in LDS crowns. TMA did not affect the fracture load of either material. However, fracture loads of ZLS crowns were lower than those of LDS crowns (p<0.001). Fracture load was moderately correlated with MG (r=-0.553) and AMD (r=-0.497). ZLS with TMA was least reliable, according to Weibull probability. CONCLUSION: Within the limitations of this study, ZLS crowns had lower fracture load values and greater marginal misfit than did LDS crowns, although these values were within acceptable limits.

The use of analogs could reduce the cost of mechanical tests involving implant-supported crowns, but it is unclear if it would negatively affect the data accuracy. This study evaluated the substitution of the implant by implants analogs or abutment analogs as a support for crowns in mechanical tests, taking into account stress distribution and fracture load of monolithic lithium disilicate crowns. Thirty lithium disilicate monolithic crowns were randomized into three groups according to the set: Implant+abutment (IA); implant analog+abutment (IAA); abutment analog (AA). The specimens were subjected to mechanical fatigue (106cycles, 200N, 2Hz) and thermal fatigue (104cycles, 5°-55°C). A final compression load was applied and the maximum fracture load was recorded. Data were analyzed using one-way ANOVA (α=0.05). The experiment was validated by finite element analysis and the maximum principal stress was recorded. No statistically significant difference was observed in the mean fracture load among groups (P>0.05). The failure mode was similar for all groups with the origin of crack propagation located at the load point application. Finite element analysis showed similar stress distribution and stress peak values for all groups. The use of implant's or abutment's analog does not influence the fracture load and stress distribution for cemented implant-supported crowns.

Current Brazilian law regarding water fluoridation classification is dichotomous with respect to the risks of and benefits for oral diseases, and fluoride (F) concentrations less than 0.6 or above 0.8 mg F/L are considered outside the normal limits. Thus, the law does not consider that both caries and fluorosis are dependent on the dosage and duration of fluoride exposure because they are both chronic diseases. Therefore, this study evaluated the quality of water fluoridation in Maringá, PR, Brazil, considering a new classification for the concentration of F in water the supply, based on the anticaries benefit and risk of fluorosis (CECOL/USP, 2011). Water samples (n = 325) were collected monthly over one year from 28 distribution water networks: 20 from treatment plants and 8 from artesian wells. F concentrations were determined using a specific ion electrode. The average F concentration was 0.77 mg F/L (ppm F), ranging from 0.44 to 1.22 mg F/L. Considering all of the water samples analyzed, 83.7% of them presented from 0.55 to 0.84 mg F/L, and according to the new classification used, they would provide maximum anticaries benefit with a low risk of fluorosis. This percentage was lower (75.4%) in the water samples supplied from artesian wells than from those distributed by the treatment plant (86%). In conclusion, based on the new classification of water F concentrations, the quality of water fluoridation in Maringá is adequate and is within the range of the best balance between risk and benefit.

Com a finalidade de correção de defeitos ósseos, diversos materiais sintéticos têm sido utilizados, entre os quais está o sulfato de cálcio. Objetivo: o objetivo do presente estudo foi avaliar, in vitro, a bioatividade do sulfato de cálcio em fluido corpóreo simulado (SBF). Métodos: quatro corpos de prova foram preparados em matrizes de policloreto de vinila (PVC) circulares, misturando-se o sulfato de cálcio nas proporções de líquido/pó recomendadas pelo fabricante, com água destilada. As amostras ficaram imersas em 50ml de SBF, a 36,5ºC, por até 21 dias, sendo que a solução foi renovada a cada três dias. A bioatividade foi verificada por meio de Espectroscopia no Infravermelho por Transformada de Fourier (FTIR). Resultados: no teste de bioatividade in vitro, a análise por FTIR detectou a presença de apatita sobre o substrato de sulfato de cálcio, demonstrando tratar-se de um material bioativo. Além disso, foi observada uma redução expressiva do tamanho da amostra vinculada ao processo de reabsorção. Several types of synthetic material have been used to correct bone defects, among which is calcium sulfate. Objective: the present in vitro study aimed at assessing the bioactivityof calcium sulfate in simulated body fluid (SBF). Methods: four specimens were prepared in polyvinyl chloride (PVC) circle matrices by mixing calcium sulfate with distilled water, as recommended by the manufacturer. Samples were immersed in 50 ml of SBF, at 36.5 ºC, for no longer than 21 days. The solutionwas renewed every three days. Bioactivity was assessed by means of Fourier transform infrared spectroscopy (FTIR). Results: The in vitro bioactivity test, carried out by means of FTIR analysis, revealed the presence of apatite formation over calcium sulfate substrate, thereby proving it to be a bioactive material. In addition, there was significant reduction in the size of the sample, which was associated with the process of resorption. Conclusion: within the limitations of the present study, it is reasonable to conclude that calcium sulfate is a bioactive material which is quickly absorbed...

Introdução: os problemas funcionais, estéticos e psicológicos decorrentes do edentulismo têm levado a uma busca crescente por reabilitação bucal com próteses sobre implantes. Esse tipo de reabilitação necessita de um planejamento meticuloso, especialmente para os casos de maxila edêntula com prótese fixa sobre implantes, em decorrência da anatomia dessa região. Objetivo: o objetivo do presente caso clínico é descrever a reabilitação de uma maxila edêntula com prótese total fixa implantossuportada, cuja infraestrutura recebeu elementos metalocerâmicos cimentados individualmente.


Introduction: Functional, aesthetic and psychological issues arising from edentulism have led to a growing search for oral rehabilitation with implant-supported prostheses. This type of rehabilitation requires detailed planning, especially for treating edentulous maxilla with implantsupported fixed prostheses due to the anatomy of this region. Objective: The aim of this case report is to describe the rehabilitation of an edentulous maxilla with implantsupported full fixed prosthesis of which framework received metal-ceramic elements individually cemented.
